Students who need certification that they have graduated from the College of Letters & Science at the University of Wisconsin-Madison can obtain assistance from the Office of the Registrar. This unit can assist students in filling out the Dean’s Form required by US law schools, medical schools, and graduate programs during the application process. Please make sure you provide your full name and UW-Madison student ID number when communicating with this unit.
The Office of the Registrar also provides information regarding disciplinary clearance that may be required by graduate, law, medical and professional schools. Additional information about disciplinary clearance can be found at dean’s certification on the Dean of Students Office website.
The Office of the Registrar can provide certification/verification letters for:
- Academic or non-academic misconduct verification**
- Academic standing letters
- Office of Student Conduct and Community Standards Certification and Recommendation Forms**
- Degree verification/completion letters once degree information has been posted to a student’s official records
- Enrollment verification
- Forms requiring a school/college Dean’s signature
- Military verification forms requiring a Dean’s signature
If you are an L&S undergraduate students who has completed all major/degree requirements but your degree has not been posted yet, please contact L&S Undergraduate Academic Deans’ Services for further assistance if you need to verify your graduation status prior to your degree getting posted to your official records.

**Students who need assistance in completing disciplinary record requests will need to work directly with the Office of Student Conduct and Community Standards. Students will need to make the request due to FERPA that protects their records. The form for students to make their record request is located at Student Resources. The form can be found on the Office of Student Conduct and Community Standards website home page by going under the Resources Tab and selecting Student.

Diplomas
Diplomas are usually mailed approximately six (6) to eight (8) weeks after L&S has notified the Office of the Registrar’s Office the names of students who have completed all of their degree requirements. Diplomas will not be mailed out to students if they still have certain types of holds on their academic records. Once the holds have been cleared, students’ diplomas will then be automatically released for mailing. See diplomas for more information.
